Following the recently published 10-year PARTNER 2 results, the current evidence can be summarized as follows:
• High-Risk/Inoperable Patients: TAVI is an established alternative to surgery.
• Intermediate-Risk Symptomatic AS (PARTNER 2): Modern TAVI matches SAVR at 10 years.
• Low-Risk Symptomatic AS (PARTNER 3 & Evolut Low Risk): Excellent outcomes through 7–8 years; 10-year durability data are awaited.
• Asymptomatic Severe AS (EARLY TAVR): Early TAVR improves 5-year clinical outcomes versus watchful waiting.
Take-Home Message
Modern TAVI has evolved from a treatment for inoperable patients to the preferred therapy for many elderly patients with severe aortic stenosis. Current evidence supports TAVI as a durable lifetime solution for most patients aged ≥80 years. Whether the same benefit extends to younger, low-risk patients will depend on the ongoing 10-year follow-up of the PARTNER 3 and Evolut Low Risk trials.
