• The ACC released a new scientific statement to improve the appropriate use of DOACs.
• DOACs are preferred over warfarin in many clinical settings because they have fewer drug interactions, predictable dosing, and a lower risk of intracranial bleeding.
• The statement emphasizes using the correct dose, regular assessment of kidney and liver function, bleeding risk, drug interactions, and medication adherence.
• Bleeding risk scores should identify patients who need closer follow-up, not be used alone to withhold anticoagulation.
• Practical recommendations are provided for patients with AF, VTE, CKD, liver disease, obesity, cancer, valvular heart disease, and around surgical or catheter-based procedures.
• Factor XI/XIa inhibitors are highlighted as promising future anticoagulants that may reduce bleeding while maintaining antithrombotic efficacy.
