• Modern mapping now provides detailed 3D visualization and precise localization of VT circuits, enabling more targeted and effective ablation.
• The current challenge is achieving full-thickness ablation of deep VT circuits to improve long-term success.
• Pulsed Field Ablation (PFA) and ultra-low-temperature cryoablation are emerging technologies being evaluated to treat VT circuits located deep within the ventricular wall.
• VT ablation remains technically demanding because patients often have advanced structural heart disease, and mapping frequently requires intentional induction of VT during the procedure.
