CCTA to Guide PCI — Key Concepts Before the Cath Lab
CCTA to Guide PCI — Key Concepts Before the Cath Lab
Source: Journal of Invasive Cardiology, October 2026 issue.
• Anatomy: Where is the stenosis, and how severe/complex is it?
• Plaque: CCTA assesses plaque burden, composition, calcium and high-risk plaque features across the coronary tree.
• FFR-CT: Is the stenosis actually flow-limiting?
• Plaque vulnerability by imaging: CCTA → non-invasive whole-coronary high-risk plaque assessment; OCT → thin fibrous cap/rupture; IVUS → plaque burden & positive remodeling (outward vessel expansion); Near-Infrared Spectroscopy (NIRS) → lipid-rich plaque.
• Key concept: CCTA = Anatomy + Plaque/High-Risk Features + FFR-CT → better PCI planning before the cath lab.
