CT-Guided PCI: A Defining Moment for Interventional Cardiology

CT-Guided PCI: A Defining Moment for Interventional Cardiology
Source: JSCAI-May01 2025.
1. What’s new?
• The Society for Cardiovascular Angiography and Interventions (SCAI) and Society of Cardiovascular Computed Tomography (SCCT) released a new expert consensus document to promote the use of coronary CT angiography (CCTA) for planning PCI procedures.
2. Why use CT-guided PCI?
• Improves understanding of coronary anatomy
• Reduces procedure time and radiation exposure
• Helps anticipate plaque morphology, vessel size, and wire path
• Already used in structural heart planning—now extending to coronary disease
3. Who benefits?
• Interventional cardiologists can better plan complex PCIs
• Patients benefit from more accurate, lower-risk procedures
4. Momentum Behind CCTA:
• CCTA now has Class 1A guideline recommendations (ESC, ACC, AHA)
• Medicare reimbursement for CCTA doubled in 2024, making it more accessible
• AI tools are emerging to support CCTA interpretation and decision-making
