• Cold increases CV risk: Higher rates of MI during cold spells (≈10%↑; prolonged cold ≈18%↑).
• Physiologic effects: Sympathetic activation → vasoconstriction, ↑ BP (15–20 mm Hg), ↑ HR.
• Heart workload: Higher BP and resistance raise myocardial oxygen demand → angina/MI risk.
• Exercise + cold: Additive BP rise, especially with snow shoveling (up to +37 mm Hg).
• High-risk groups: ASCVD, heart failure, older adults, low fitness.
• Key advice: Dress warmly, avoid strenuous outdoor exertion, limit alcohol.
• Red flags: Chest pain, dyspnea, left arm/back pain, excessive sweating → seek care.
