From Radiology to the Bigger Picture – 2025 Overview
1. Radiology’s Experience with Signaling
• In diagnostic radiology, a new tool called preference signaling was introduced two years ago. This allows applicants to tell certain programs, “You’re one of my top choices.”
• The idea is to make applications more personal and targeted, instead of sending the same generic application everywhere.
• Results have been strong: when signaling was combined with an away rotation (short-term training at the target program), 70% of applicants matched there. Without signaling, only 8% matched at a program where they had rotated.
• Lesson: In competitive specialties, carefully choosing your targets and showing interest directly can work much better than applying to dozens of places without a plan.
2. Internal Medicine – Residency Situation (2025)
• Positions Offered: 11,750 (6.1% more than in 2024).
• Positions Filled: 11,379.
• Fill Rate: 96.8% — meaning almost every available spot was taken.
• What it means: Internal medicine is one of the largest and most stable specialties in the U.S. For qualified applicants, the chances of getting a spot are high compared to other fields.
3. Pediatrics – Residency Situation (2025)
• Positions Offered: 3,193 (54 more than in 2024).
• Positions Filled: 3,043.
• Fill Rate: 95.3% — better than 2024’s 91.8% but still a little below the pre-2024 level of 97.1%.
• What it means: Pediatrics also has a strong match rate, but the numbers can vary more from year to year compared to internal medicine.
4. Cardiology – Fellowship Situation (Latest Data)
• All Fellowship Specialties (2025): 14,620 positions offered, 12,390 matched → Overall Match Rate: 83.5%.
• Cardiology (CVD, 2024 data):
• Match Rate: ~66.3% — meaning about one in three applicants did not match.
• Fill Rate: 99.8% — almost every cardiology spot was taken.
• What it means: Cardiology is much more competitive than internal medicine or pediatrics. Many applicants don’t get in, even though nearly all positions are filled.
• Possible strategy: Borrow from radiology’s success — use targeted program outreach, do away rotations, and focus on programs where your skills and experience fit well.
What does “Fill Rate” mean?
Fill rate is the percentage of available training spots that get taken in the match. A high fill rate (like 95–100%) means nearly all positions are filled — whether the match rate is high or low.
Final Advice
If you’re aiming for a very competitive specialty like cardiology, don’t just send more applications — send better ones. Choose programs carefully, build real connections through rotations, and clearly communicate your interest. This approach has proven results in radiology and can be adapted to other specialties.

If the same applicant with the same qualifications applies to Diagnostic Radiology, Internal Medicine, and Pediatrics in 2025, the acceptance likelihood from highest to lowest would be:
1. Internal Medicine
• Fill Rate: 96.8%
• Very high acceptance likelihood because it is one of the largest specialties with a large number of positions compared to applicants.
2. Pediatrics
• Fill Rate: 95.3%
• Slightly lower than Internal Medicine, with fewer total positions and some fluctuation in fill rates, but still generally accessible for qualified applicants.
3. Diagnostic Radiology
• Match Rate: ~81% for U.S. MD seniors in 2023 — the most competitive cycle since 2001.
• Fewer positions than Internal Medicine or Pediatrics, and highly competitive. Acceptance chances are lower without strong credentials or targeted strategies like preference signaling and away rotations.
🔍 Bottom line:
With all other factors being equal, Internal Medicine offers the highest chance of matching, followed by Pediatrics, while Diagnostic Radiology is the most competitive and offers the lowest acceptance probability.
The smartest way to improve your chances is not to apply everywhere, but to apply where you really fit best.
This means choosing programs in places you know, where you’ve studied, worked, or done a rotation — and making that clear in your application.
In every part of the form, show why you truly want that program, what makes you a strong candidate for them, and how your skills match their needs. This personal, targeted approach works much better than sending the same generic application to many programs.
