Case Overview:<\/span><\/div>\n<div>A 22-year-old college football player collapsed after sprinting drills in hot, humid weather (90\u00b0F, 85% humidity). He reported muscle pain, weakness, dizziness, and confusion.<\/div>\n<div><span> 2. Event Details:<\/span><\/div>\n<div>The collapse occurred about 45 minutes before arriving at the ER. He was performing repeated 50-yard sprints, sweating heavily, and likely dehydrated.<\/div>\n<div><span> 3. Medical History:<\/span><\/div>\n<div>Generally healthy; only prior injury was a left ankle sprain. No drugs or supplements. Drinks alcohol socially. Family history of diabetes and hypertension.<\/div>\n<div><span> 4. Physical Exam:<\/span><\/div>\n<div><span> \u2022 Temp 99.5\u00b0F, BP 116\/74 mmHg, HR 110 bpm<\/span><\/div>\n<div><span> \u2022 Muscular but obese (113 kg, 1.88 m)<\/span><\/div>\n<div><span> \u2022 Muscles tender and \u201cdoughy,\u201d proximal weakness (4\/5 strength)<\/span><\/div>\n<div><span> \u2022 Neurologic exam normal, no focal deficits.<\/span><\/div>\n<div><span> 5. Lab Findings:<\/span><\/div>\n<div><span> \u2022 CK = 12,976 IU\/L (\u2191\u2191; normal 20\u2013200)<\/span><\/div>\n<div><span> \u2022 Creatinine = 1.7 mg\/dL (slightly high \u2192 renal stress)<\/span><\/div>\n<div><span> \u2022 Urinalysis: positive for blood but no RBCs \u2192 myoglobinuria<\/span><\/div>\n<div><span> \u2022 Electrolytes and calcium normal; WBC mildly elevated (11,200 \/\u03bcL).<\/span><\/div>\n<div><span> 6. Probable Diagnosis:<\/span><\/div>\n<div>Exertional rhabdomyolysis \u2014 muscle breakdown due to intense exercise in heat, leading to myoglobin release and risk of acute kidney injury.<\/div>\n<div><span> 7.
