EuroPCR 2026: New Balloon-Assisted Technique For LVOT Protection Before TMVR
* Two major techniques are now used to reduce LVOT obstruction risk in TMVR:
* LAMPOON — current electrosurgical anterior leaflet-splitting technique
* BATMAN — newer balloon-assisted anterior leaflet-splitting technique designed to create more LVOT space in TMVR
* BATMAN may offer a simpler and faster alternative to LAMPOON, with:
* 99% procedural success
* No procedural deaths
* Why the anterior leaflet?
Because it can be displaced toward the LVOT during TMVR and obstruct blood flow from the left ventricle.
* Particularly useful in:
* Severe MAC
* Small predicted LVOT space
* Related concept:
* BASILICA = similar catheter-based leaflet-splitting procedure, but used in TAVR/TAVI to prevent coronary obstruction.
* Key concept:
“Prepare the anatomy before implanting the valve.”
