• New concept in TAVI patients with CAD:
• Treat the aortic valve first
• Reassess coronary lesions later if needed
• Routine PCI before TAVI is no longer always necessary.
• Many patients improve after TAVI alone and may never need PCI.
When Should PCI Be Done First Before TAVI?
• In unstable patients, revascularization first—or a combined strategy—may still be necessary.
