GuideLiner: The Ultimate Guide Extension for Complex PCI

When a balloon or stent simply will not cross a lesion despite optimal guidewire position, the GuideLiner can be a true game changer.

What is it?

The GuideLiner is a guide extension catheter (“mother-and-child” system) that advances beyond the tip of the guiding catheter into the coronary artery, providing additional backup support for device delivery.

Common Indications:
Severe calcification
✅ Tortuous anatomy
✅ CTO
✅ Difficult balloon or stent delivery

Practical Tips

•⁠ ⁠Advance gently under fluoroscopic guidance.
•⁠ ⁠Ensure continuous flushing to prevent air embolism.
•⁠ ⁠Avoid excessive deep intubation, particularly in small or diseased vessels.
•⁠ ⁠Consider Balloon-Assisted Tracking (BAT) if advancement is difficult.

Potential Complications
⚠ Coronary dissection
⚠ Transient pressure damping or ischemia
⚠ Proximal stent deformation or stripping during delivery
⚠ Air embolism

Clinical Pearl

💡 The GuideLiner does not increase the size of the guiding catheter—it extends backup support exactly where it is needed. It is particularly valuable when the guidewire has crossed the lesion successfully, but balloons or stents fail to cross using conventional techniques.
