US Medicare (CMS), the Government Health Insurance Program, Proposes Expanding TAVI Coverage to Asymptomatic Patients

• CMS has proposed expanding Medicare coverage to include patients with asymptomatic severe aortic stenosis, while continuing to require Coverage with Evidence Development (CED) for this group, while maintaining Heart Team evaluations(multidisciplinary team decision-making).
• For symptomatic severe aortic stenosis, CMS proposes removing the CED requirement, potentially improving access to TAVI.
• The proposal was driven by the EARLY TAVR trial, which showed better outcomes with early valve replacement than with watchful waiting in asymptomatic patients.
• The proposal is open for public comment until July 15, 2026, with a final CMS decision expected in September 2026.
