Modern TAVI Matches SAVR at 10 Years in Intermediate-Risk Patients

•⁠ ⁠Surgical risk is determined using the STS (Society of Thoracic Surgeons) score, which classifies patients as low, intermediate, or high risk based on their predicted operative mortality.

•⁠ ⁠Intermediate-Risk Patients (PARTNER 2, 10-Year Follow-up): Modern TAVI achieved mortality and valve reintervention rates comparable to surgical aortic valve replacement (SAVR) in patients with symptomatic severe aortic stenosis at intermediate surgical risk, highlighting the impact of improved valve design and CT-guided procedural planning.

•⁠ ⁠Low-risk symptomatic patients: Excellent 5–8-year outcomes have been demonstrated (PARTNER 3 and Evolut Low Risk), but 10-year durability data are still awaited before confirming TAVR as definitive lifetime therapy for younger patients.
•⁠ ⁠Asymptomatic severe aortic stenosis: EARLY TAVR demonstrated that early intervention improved 5-year clinical outcomes compared with watchful waitin

•⁠ ⁠Take-Home Message: Current evidence supports modern TAVI as a durable lifetime solution for most patients aged ≥80 years. Whether the same benefit extends to younger, low-risk patients will depend on the 10-year results of the PARTNER 3 and Evolut Low Risk trials.
