Trigger-Induced Atrial Fibrillation (AF): Key Clinical Messages

•⁠ ⁠Not all new-onset AF is primary AF. Acute or chronic reversible conditions may trigger the arrhythmia.
•⁠ ⁠Always search for the underlying trigger rather than focusing solely on rate or rhythm control.
•⁠ ⁠Common acute triggers include:
* Sepsis
* Pneumonia and other infections
* Urinary tract infection
* Dehydration
* Electrolyte disturbances
* Acute myocardial infarction
* Surgery
* Hyperthyroidism
•⁠ ⁠Common chronic triggers include:
* Obesity
* Obstructive sleep apnea
* Hypertension
* Diabetes mellitus
* Heart failure
* Inflammatory bowel disease
* Excess alcohol consumption
•⁠ ⁠A focused history and physical examination are essential to identify potentially reversible causes.
•⁠ ⁠Anticoagulation should be based primarily on CHA₂DS₂-VASc (CHA₂DS₂-VA in ESC 2024), with individualized consideration of bleeding risk and whether the AF episode was trigger-induced.

Take-home message

Trigger-induced AF should be viewed as a clinical warning sign rather than an isolated arrhythmia. Identifying and treating the underlying trigger, together with long-term cardiovascular risk management, is essential to reduce recurrence and improve patient outcomes.
