A1c + BP Time in Range & Kidney Outcomes | AJKD 2026

* Post-hoc analysis of ACCORD, including 6,542 patients with T2D and high CV risk.
* Focus: maintaining A1c and systolic BP (SBP) within target over 1 year, rather than relying on single measurements.
* A1c 100% in target range: associated with 20% lower risk of the composite kidney outcome.
* SBP 100% in target range: associated with 32% lower risk.
* Both A1c + SBP in target >80% of the time: associated with 29% lower kidney risk and 43% lower all-cause mortality.
* Kidney composite: new albuminuria, ≥40% eGFR decline, or kidney failure.
* Key message: In diabetes + hypertension, consistent long-term control of both glucose and BP may matter more than an occasional “good” reading.
* Important: This was a post-hoc observational analysis of trial data → associations, not proof that increasing time-in-range itself causes these benefits.
