* Analysis of ~500,000 TAVR patients from 808 U.S. hospitals (2017–2023).
* High-volume centers had lower rates of major complications and 30-day mortality than low-volume centers.
* However, failure-to-rescue (FTR)—death after a major TAVR complication—was similar across high- and low-volume hospitals (overall FTR ≈11%).
* The volume–outcome advantage appears to be driven by preventing complications, not by better management once complications occur.
* Clinical message: Improving prevention of major TAVR complications, including AI-assisted preprocedural planning and adherence to best practices, may help lower-volume centers achieve outcomes closer to experienced high-volume programs.
