* Routine IVUS-guided PCI did not reduce major complications related to the treated coronary artery compared with angiography-guided PCI in complex high-risk PCI (13.9% vs 11.1%).
* IVUS guidance resulted in longer procedures and more post-stent balloon dilatation, without reducing procedural complications.
* Clinical message: Routine IVUS use for all complex high-risk PCI is not supported. IVUS should be used selectively when intravascular imaging is expected to optimize stent sizing, expansion, apposition, or identify procedural complications.
