First RCT suggesting benefit of DOACs in intermediate-risk AF
• 1,803 AF patients with intermediate stroke risk (CHA₂DS₂-VASc 1 men / 2 women).
• DOACs ↓ clinical events 69%: 0.5% vs 1.5% at 2 years (HR 0.31).
• Stroke: 0.3% vs 1.1%; no increase in major bleeding.
• Caution: Very few events; benefit may be overestimated → further trials needed.
