• 508 endocarditis patients: tailored antibiotics 26 vs 41 days.
• Noninferior safety: 8.2% vs 10.7%.
• More relapse: 5.1% vs 1.6%, especially E. faecalis.
• Message: Selected stable patients may safely receive shorter antibiotic therapy.
