(New JACC Study Highlights the Potential of Focal PFA for PVC Ablation)
Key Findings :
• Prospective multicenter European study of 80 patients treated with focal pulsed field ablation (PFA) for symptomatic outflow tract premature ventricular contractions (PVCs).
• Acute procedural success: 100%.
• Mid-term success: 88% at a median follow-up of 8 months.
• Success rates by location:
• RVOT: 94%
• Endocardial LVOT: 89%
• LV Summit: 73%
• Among patients with previous failed radiofrequency ablation (RFA), 93% remained free of recurrence.
• Major complication: 1 ischemic stroke (1%).
• Coronary vasospasm occurred in 17% of patients undergoing coronary venous ablation and resolved with nitroglycerin.
Take-Home Message:
• Focal PFA demonstrated excellent acute success and encouraging mid-term outcomes for outflow tract PVCs, particularly in redo cases after failed RFA.
• Outcomes were strongest for RVOT and endocardial LVOT PVCs, while LV summit and intramural arrhythmias remain more challenging.
