* Traditionally contraindicated in severe aortic stenosis (AS) because of concerns about hypotension.
* Prospective study (109 patients) found 0.8 mg sublingual nitroglycerin before pre-TAVI CCTA was safe and well tolerated in carefully selected patients.
* Significantly improved coronary artery image quality and visualization.
* Associated with a 27% relative reduction in invasive coronary angiography (not statistically significant).
* Mean systolic BP decreased from 158 to 140 mmHg, without serious hemodynamic instability
* Patients with baseline hypotension, LVEF <30%, critical AS, or other contraindications were excluded.
* Investigators concluded that routine use of sublingual nitroglycerin before pre-TAVI CTA may be appropriate in clinically stable patients with severe AS.
