EuroPCR 2026: BATMAN (Balloon-Assisted Translocation(slpitting) of the Mitral Anterior Leaflet) Expands TMVR Options in High-Risk Patients
• * Two major techniques are now available to reduce LVOT obstruction risk before Transcatheter Mitral Valve Replacement(TMVR):
• LAMPOON
• BATMA
• LAMPOON: Intentional “electrosurgical” splitting of the anterior mitral leaflet before TMVR to reduce LVOT obstruction.
• BATMAN: “Balloon-assisted modification/splitting” of the anterior mitral leaflet before TMVR to create more LVOT space.
• BASILICA: Intentional leaflet laceration before “TAVR= TAVI” to prevent coronary artery obstruction.
• BATMAN is a balloon-assisted technique used before TMVR to open/split the anterior mitral leaflet and create more LVOT space.
• Why the anterior leaflet?
Because it can be pushed toward the LVOT during TMVR and obstruct blood flow from the left ventricle.
• Registry data showed:
• * 99% procedural success
• No procedural deaths
• Especially useful in:
• Valve-in-ring
• Severe mitral annular calcification (MAC)
* Small predicted LVOT space
• BATMAN may offer a simpler and faster alternative to LAMPOON.
• Key concept:
“Prepare the anatomy before implanting the valve.”
