Expanding the Window for Thrombectomy in Large-Vessel Ischemic Stroke

The randomized LATE-MT and MILD-MT trials demonstrated that mechanical thrombectomy improves outcomes in stroke patients with acute ischemic stroke due to large-vessel occlusion (LVO) — including some presenting up to 72 hours after symptom onset and even patients with mild neurological deficits — provided CT perfusion or MRI perfusion imaging confirms salvageable brain tissue.
• Mechanical thrombectomy is already an established standard therapy for patients with moderate-to-severe neurological deficits due to LVO; these trials further expand potential eligibility to carefully selected late-presenting and mildly symptomatic patients.
